    -Account Number
    -Card Type   (V,MC or Am.EX)
    -Card Expiration Date
    -Name of card holder

  If you are a California resident please add 8.00% sales tax. U.S. 
  residents add $6.00 shipping and handling fees and International users 
  add $8.00.  Orders will be returned without the proper fees.

  Postal Mail Registration:  You may send cash, Money Orders, or checks
